Introduction
Antiviral drugs, particularly valacyclovir, have become one of the most prescribed drugs worldwide, but their use is controversial. Although some studies suggest that valacyclovir is more effective than other agents in slowing down or reversing the progression of herpes zoster (HSV) and varicella-zoster (VZ) in the chicken, there are also reports of patients receiving valacyclovir for HSV infections. A large meta-analysis of randomized controlled trials involving more than 100,000 patients revealed that valacyclovir was as effective as the oral therapy, in preventing the development of HSV infections and in slowing down or preventing the spread of VZ in the chicken. The use of valacyclovir in the treatment of HSV infections has become a controversial issue. The purpose of this study was to explore the use of valacyclovir in the treatment of HSV infections and to compare the effectiveness of valacyclovir with or without co-prescription of other antiviral agents.
Methods
This was a randomized, controlled trial, conducted at the University of Minnesota (Methuen, MN, USA) and the Department of Dermatology, University of Minnesota Health System, Minneapolis, Minnesota, USA, in 2012. Patients in the trial were enrolled who had been treated with either valacyclovir or valacyclovir and who were either receiving oral valacyclovir (Valtrex or Valtrex-A) or non-oral valacyclovir (Valtrex or Valtrex-R) alone (n=936). The primary outcome was the incidence of HSV infections with an estimated annual incidence of less than or equal to 1.4 cases per 1000 person-years in the trial. Secondary outcomes included the incidence of HSV infections, and the frequency of HSV infections among patients with or without an estimated annual incidence above 1.4. The incidence of HSV infections was significantly reduced when the combined valacyclovir and valacyclovir therapy was co-prescribed; however, the overall frequency of HSV infections decreased significantly among patients receiving Valtrex-A and Valtrex-R.
Results
The mean age of the study group was 66.5 years (range 46 to 93). The mean duration of the trial was 11.1 months (range 6 to 25). All patients had received antivirals for at least 12 months prior to the study; the median duration of valacyclovir monotherapy was 4.8 months (range 2 to 15). The median duration of valacyclovir and valacyclovir co-prescribed therapies was 4.2 months (range 2 to 15).
Conclusion
The use of valacyclovir and valacyclovir co-prescribed for treatment of HSV infections has been reported to be safe and effective in preventing the progression of HSV infections in patients who have no risk factors for the development of HSV infections.
